Mission Choices' mission is to empower individuals in the Mid-South community to make informed choices for and about their reproductive health.
Background Choices, an independent, non-profit community health agency (formerly known as Memphis Center for Reproductive Health) was founded in 1974 by Priscilla Chism. The agency's mission is to empower individuals in the Mid-South community to make informed decisions for and about their reproductive health. From the beginning, the agency provided innovative services including free pregnancy testing, first trimester abortion care, and counseling that was specifically focused on the needs of the partners of abortion patients. In addition, Choices helped establish the first program in the United States to provide counseling, gynecology, and forensics services around the clock to victims of sexual assault.Choices is now working to build a more comprehensive reproductive medical practice that provides a range of sexual and reproductive health services for more than 3,000 women, men, and teens each year. Located in a beautifully renovated facility on Poplar Avenue in Midtown, Choices offers a new model for community-based, comprehensive, reproductive health care.
Impact The goal of our independent, non-profit clinic is to completely transform the way reproductive health care is perceived and provided in our community. Choices commitment to a patient-centered practice is one of the distinguishing characteristics of the feminist model of health care, and we are proud to continue providing comprehensive reproductive health services in a safe environment and with respect for individual beliefs.
Needs
Choices is also seeking support to open the first truly comprehensive non-profit reproductive health center in the nation. Choices relies on donations to fund our patient assistance fund which we use to supplement payments for women, men, and teens who are uninsured or under insured and can not pay for services.
 
With the opening of the new center, which will include the first licensed birth center in Memphis, TN, Choices will become the first non-profit health care provider in the country to offer birth services and abortions along with mental health care in one comprehensive care center. The future Choices Birth & Family Center will provide prenatal, ante-partum, and post-natal care, childbirth education, parental education, and birth services. CBFC will offer birthing services in a homelike, relaxed, private, and culturally sensitive environment, attended by trained physicians, midwives, and doulas. In addition to an OB/GYN and Certified Nurse Midwives, the Center will employ a Director of Counseling, Education and Patient Services who will provide pre- and post-natal mental health services and education for mothers, and will assist expectant mothers in enrolling for TennCare and other eligible support services.

Description
Choices offers a wide range of medical services all revolving around comprehensive reproductive health and education. We believe everyone has the right to sexual health and reproductive autonomy - and the right to make informed choices.We are committed to offering reproductive health care in a positive, patient-centered, and supportive environment. Services include birth control, abortions, the morning after pill, HIV testing, STD testing, PAP smears and gynecological services, pregnancy testing, and fertility testing. 
 
In 2019 Choices will open the first in the nation comprehensive reproductive health center providing all the above services and including prenatal care, midwifery led birth services, and mental health.

Short Term Success Choices has seen a significant increase in donations, sponsorship, volunteers, and overall visibility in the Memphis community in the last year. 
Long Term Success In the past few years we have experienced tremendous growth in services, visibility, and impact. From 2005 to 2015, Choices has seen a huge increase in expenses and revenues. The budget in 2015 was 3X the size of the budget from 2005 and the same can be said about the revenues. Abortion numbers have been decreasing nationally, and it has been especially evident of this trend at Choices. This is most likely due to the Memphis-wide program revolving around free LARCs, or long lasting reversible contraception. As abortion numbers decrease, the number of wellness exams performed at Choices has been on the rise, almost at the same rate as the decrease in abortions. 
Description
Choices is proud to provide comprehensive, non-judgemental sexual and reproductive health services to everyone, including the LGBTQ community. Our goal is to provide this community with all the information and resources they need to protect their health and make informed decisions. Choices respects any sexual health decisions made and the need for privacy and confidentiality. We are a beacon of hope for many LGBTQ people in our area. Whether it is through fertility treatment, adoption referrals, STI testing and treatment, or transgender hormone therapy, we provide care and services that many of our patients are unable to receive anywhere else. Choices is also the only clinic in the Memphis area to offer comprehensive transgender healthcare.

Long Term Success Through our close relationship with the Memphis Gay and Lesbian Community Center (MGLCC) and other LGBTQ nonprofits, Choices has become a leader in LGTBQ healthcare in the Memphis community. Our increase in services and visibility has made Choices an amazing resource for the LGBTQ community.
Description At Choices, we trust women and men to know when becoming a parent or creating a family is right for them. For this reason, Choices is excited to offer the Choices Fertility Program. Our Fertility Clinician specializes in offering services to clients in our community who may be seeking a pregnancy as a single individual, as a same sex couple, or for individuals and couples who would prefer fertility assistance that supports a more natural conception and individual choices of treatment modalities. Sero-discordant couples (one partner is HIV+ and one is negative) are not excluded from applying for the Fertility Program.Clients who have been accepted in to Choices Fertility Program are eligible to access our complete menu of fertility assistance procedures including: basic sperm analysis, intra-uterine insemination, sperm washing, intra-cervical insemination, guidance in purchasing donor sperm, management of known donor insemination, and support for self-insemination or home insemination.

Description

 CHOICES received special grant funding to provide FREE long-acting reversible contraceptives (LARCs). The program is available to anyone, insured or uninsured. No asset tests or financial qualifications are required.

Short Term Success Visibility and promotion of this program has increased in the last year and the number of LARCs inserted/provided has increased as well. 
Long Term Success This program has had a huge part in the decrease of abortion rates at Choices and has increased the visibility and access to birth control in the Memphis community. 
Description This project seeks to increase awareness, knowledge, and collaborations among reproductive health and HIV/AIDS service providers in order to ensure the sexual health needs of women in Memphis/Shelby County are met in a comprehensive manner. The goal of this project is to connect and integrate HIV/AIDS and reproductive health services in Memphis/Shelby County, TN.

Short Term Success This project has created several service and promotional opportunities for MCRH. MCRH has strengthened collaboration with many local HIV/AIDS providers, and the number of referrals from these providers has increased dramatically. In November 2010, MCRH staff was asked to provide a case study of our HIV/AIDS services integration for a report to be published by the ATHENA Network and UNAIDS.   MCRH is also working in collaboration with U.S. Positive Women's Network of WORLD, another MAC AIDS Fund grantee, to design and implement a reproductive health needs assessment of HIV positive women in Memphis, TN, Oakland, CA, and Washington DC in 2011 with the long term goal of designing a national blueprint for the integration of HIV/AIDS, reproductive health, and domestic violence services for women nationally. It is the goal of all collaborators to release the results of the needs assessment and the national blueprint at that International AIDS Conference in Washington, DC in 2012.
Long Term Success For over 20 years, reproductive health and HIV/AIDS providers have been working on parallel, yet separate paths. Women's needs related to HIV and reproductive health care often inextricably linked. Not only do reproductive health clients need to understand their risks for HIV and take these risks into account when making decisions about their sexual and reproductive health, but HIV-positive women have pregnancy prevention and planning needs as well. The integration of reproductive health and HIV works to fully meet the reproductive health needs of women, and offers them more comprehensive information for decisions making. Reproductive health providers also have access to many women who may never seek the services of an HIV/AIDS provider. Recognizing the increasing number of HIV infected women and the heterosexual transmission pattern, MCRH is seeking to connect and integrate reproductive health and HIV/AIDS services in Memphis/Shelby County.
Examples of Program Success All Choices staff participated in HIV/AIDS Starter Facts. Also, all direct patient care staff participated in an abbreviated version of the Fundamentals of HIV Prevention Counseling. As of January 31st, Choices provided rapid HIV testing to 2271 of our patients and 273 patient partners and community members for a total of 2544. HIV prevention counseling and rapid testing have become routine parts of patient interactions and literature in the clinic. From July to August 2010, Choices facilitated a 12-week session of reproductive health education with 15 HIV-positive women.  From August 2010 to February 2011, Choices hosted 5 lunch and learn sessions that were 1 to 2 hours each for a total of 9 hours of reproductive health education. These sessions were geared for HIV/AIDS medical and social service providers and focused on topics of reproductive health in relation to HIV/AIDS.  A total of 52 HIV/AIDS medical and social service providers, representing 11 HIV/AIDS services agencies, attended the sessions.
